New Patients

We accept registrations from patients residing in Stevenage and the surrounding villages.

Patients can now register with our practice directly online, please click the link below:

Alternatively, you can download the forms below and bring them to either site, or visit either practice to collect registration forms. Please ensure you complete both the Registration Document and GMS1 form, for children please complete a GMS1 and Child Registration Form.

Once completed, return forms to either site.  Registration usually takes up to seven working days.  If you are bringing forms to the practice, please attend after 11am where possible to allow our reception team time to check through the forms before submission.  8am to 11am can be an exceptionally busy period and you may be asked to wait – thank you for your assistance in this matter.
