We are changing the way we monitor long term conditions at the practice. Going forwards, we would like to invite all patients over 55 years of age with long term medical conditions to come for an annual face-to -face review with a GP. This will be an ongoing process, and we will invite you every year in your birth month.
Prior to the appointment, we would like you to have a blood test; visit https://www.enherts-tr.nhs.uk/services/blood-tests/ select Lister Hospital, and book an appointment. You need to set up an account with SwiftQueue which is easy to do. You can also call 01438 284044 to book if you are not able to use the online link.
We do have some capacity for blood tests at the practice – please call us to book.
If you have had your bloods done recently (since February 2023) you do not need to have another blood test. If you do need a blood test, it has been requested for you – you just need to arrange an appointment at Lister Hospital.
Once you have had your blood test please contact us to book your review appointment – please let the Receptionist know you are requesting a ‘long term condition review‘.
We are specifically creating extra patient capacity for these important checks, appointments will be routine and bookable a few weeks in advance. It would really assist us if you could call in the afternoon to make your appointment – phone lines are particularly busy in the mornings.
It would be helpful if you attend the appointment with at least 3 home blood pressure readings taken in the preceding days, if you are able to access a monitor at home. Monitors are reasonably inexpensive, and can be purchased from many pharmacies or online from Amazon etc. The British Heart Foundation has some information and links on their website, click here
