Register for the Electronic Prescribing Service (EPS)

If you are not an eligible dispensing patient, did you know you can nominate your preferred local pharmacy to receive your repeat prescriptions? Simply notify us of your preference, and we will send your prescription electronically, saving you time – you will not need to collect your prescription from the surgery. Ask at reception or call us.

How to Order Repeat Prescriptions

Requests may be made using your computerised prescription counterfoil.

  • using the letter box outside the surgery near the entrance
  • using the letter box inside the entrance foyer
  • by post, enclosing a stamped addressed envelope

If you do not have the computerised counterfoil you may download this repeat prescription form and hand it into reception or post it to the surgery.

Please allow two working days before collection (see below chart for more details).

Prescriptions may be dispensed for our ‘eligible’ patients (those who live more than one mile from a chemist, and live in an area of rural character).

The dispensary is open from 8.30am to 6.00pm Monday to Friday (please note that the dispensary is CLOSED on Wednesdays between 12.30pm and 1.30pm and Saturdays).

Asthma Review

  • Please give us your full name

  • (including cough)
  • (cough, wheeze, chest tightness or breathlessness)
  • (housework, work, school etc)
  • If you have answered YES to any of the above questions, it is very important that you make an appointment to have a face-to-face review with a practice nurse. We may need to change your medication. If you have answered NO to all three questions, we will complete your asthma review for the year, however in certain circumstances we may contact you by phone to discuss your medication and whether it should be changed or stopped. Please note the details you have provided will be used to update your records, and should your symptoms worsen at any time you should contact us.

    I understand if I consent to share all of my health information - this includes my current health conditions, my current medications, any allergies I may have, any recorded information regarding my family relationships (i.e. my next of kin) and information about my decisions regarding my preferred place of care.
  • Please provide your mobile number. Providing this will be seen as receipt of your consent to record this number on your medical records, and for its use for your direct medical care only (i.e. text message reminders for appointments etc)
